 Athletic Forms

  • ALL student-athletes are required to get an EKG along with the physical to be eligible to participate in athletics. 
  • Physicals are valid for one season of practice and competition only, sports physicals can be performed by your primary care physician
  • Athletic Trainers in Arizona practice under the direction of either MD or DO. Physicals can be accepted and signed by MD, DO, NP, or PAs. We do NOT accept physicals signed by chiropractors (DC) or naturopathic doctors.
  • The packet below needs to be printed, completed and submitted by student-athletes to the SCC Athletic Training Office prior to any preseason conditioning or in-season activities.

Athletic Training Eligibility Documents

Visiting student-athletes are required to sign and submit the form below along with a valid physical (no more than a year old) in order to participate in tryout/walk on practice sessions.

General Assumption of Risk & Release of Liability Document 

 

 Insurance 

SCC carries a secondary insurance on all eligible student-athletes. This policy is secondary only, with medical insurance of parents/guardians/self being primary insurance. The insurance policy only covers athletic injuries that must have occurred during MCCCD sponsored game or practice. All injuries must be reported to athletic trainer, coach, or athletic director when they occur. The college's insurance carrier does not accept responsibility for gen-med illnesses, or pre-existing injuries or conditions.

MCCCD Supplemental Accident Insurance Guidelines:

  • Injury must have occurred while participating in official MCCCD competitions, games, or practices
  • Report injury in a timely manner to a MCC athletic training staff member and fill out an Accident Report/Insurance claim. THIS MUST BE DONE WITHIN 90 DAYS OF INJURY
  • Submit all itemized medical bills to your personal/primary insurance carrier FIRST.
  • Submit all copies of all itemized medical bills and your primary insurance's Explanation of Benefits (EOB) forms to MCCCD's supplemental insurance carrier (address below)
  • Keep a personal copy of all medical and insurance related paperwork.

 Visiting Team Information

On behalf of our athletic training staff, we are happy to assist your athletes. If your team is traveling here without an athletic trainer, and your athletes require specific taping and treatment needs, please contact our staff prior to your team's arrival. Visiting teams are responsible for sending their own supplies as well as treatment and modality scripts.
